Additional Services:
Prescription Refills: You or your pharmacy may phone in prescription refills to either office. Press extension 18 to reach our refill line. Please allow 24 hours for refills.
Allergy Serum Refills: Please mail or fax refill requests directly to the lab in the Mclean office. The fax # is 703-356-8031. The mailing address is 6888 Elm St. #301, Mclean, VA 22101. All requests should have a signed consent form, a current allergy shot record and a current insurance referral if required by your insurance company. If you wish to get your first shot in our office when picking up a refill, your referral must have 2 visits. You will receive a note in the mail when your serum is ready for pick up. Note: It is our office policy that no allergy shots will be given on the same day as an office visit with the physician.
School/Camp forms/Letters: Due to the
amount of time necessary to complete forms and letters, there will be a charge
for these forms. Forms and letters require more than a signature - charts must
be reviewed to verify medications, diagnosis and compliance with recommendations
and follow-ups.
The charges for each form/letter are as follows. Please allow 5 business days to
process requests. Request to have forms completed in less than 5 business days
will incur a $10 surcharge.
$20 for
the first form, $10 for each additional form per patient.
$25 for
all letters (this includes any letters written by the provider to any
school or insurance company and/or letters regarding medical necessity.
Payment must be received before letters can
be mailed, faxed or picked up. If you would like to include payment with
your school form, please make the check payable to Allergy and Asthma
Associates.
